1. Click on SUBMIT AN INCIDENT located at the top right corner of the page and at the Menu Bar. The link to the form is also located in the lower box named HOW TO REPORT.

2. Enter a short title of your report.

3. Describe the incident by providing four Ws and one H. (Who, what, Where, When, and How).

4. Select the category that best describes the incident you are reportingby.

5. If you prefer, you can enter your name and email address so we can confer with you about the incident.

FIND A LOCATION NEAR YOU

6. Select a city/town or place where the incident happened. If it's not there, (a) choose the nearest city/town where the incident occured, or (b) find the place in the interactive map by clicking the name of the city/town or (c) the city/town where the incident happened, or (d) type the city/town in the box FIND LOCATION.

7. If you have a picture or a video of the incident you are reporting, you can upload it by clicking on the UPLOAD PHOTOS or VIDEO LINK boxes.

8. Click on SUBMIT. Done!

You can also submit a report via email (cleanelections2010@vmafoundation.org) Please include the answers to the four Ws and one H. (Who, What, Where, When, and How).

The Map

The color dots on the map represent an incident (see ALL CATEGORIES). Reports come from observers on the ground like you or lifted from the newspapers, TV, or radio broadcasts.
